But this work is not destined to remain on a bookshelf. In these times of digital communication, it would be
a shame if the Textbook would simply be a printed book. The online edition goes beyond the print version
giving the reader access to the archives of EAPCI and PCR with links to recorded cases, slides, and
manuscripts related to the topics within the print edition. Thus, this online edition opens our collective
memory, using previous experiences and thoughts, linking theory to practice, and converting general
